Course Detail: OHFA - HFACS - Human Factors Analysis and Classification System (Virtual)
Master human error management and acquire advanced skills to analyze and mitigate human factors, enhancing safety and performance across diverse industries.
Led by Dr. Scott Shappell & Dr. Douglas Wiegmann
Overview
The Human Factors Analysis and Classification System (HFACS) is the global benchmark for in-depth and credible analysis of human factors in adverse events. Widely applicable in accident investigation, risk reduction, safety, and productivity enhancement, HFACS is now being offered through specialized training by Embry-Riddle Aeronautical University in collaboration with HFACS, Inc.
With human error contributing to 60 to 80% of safety failures, injuries, and quality defects across industries like aviation, healthcare, mining, and manufacturing, mastering its management is crucial for organizational profitability and success.
This intensive 2-day HFACS Course will equip you with scientifically validated methods for transforming errors into actionable insights and effective solutions.
The HFACS Course is also available in-person and held on the Daytona Beach Campus, Florida and in Las Vegas, Nevada.
The HFACS Course can be completed independently, or toward the Certificate of Management in Aircraft Accident Investigation.
Additionally, the HFACS-Advanced Course - available virtually - offers advanced training on proactive safety tools and data analytics, making it a valuable follow-up to the core HFACS Course.
You will learn how to:
- Integrate human factors and system safety concepts into the root cause analysis (RCA) or accident investigation process.
- Apply the Human Factors Analysis & Classification System (HFACS®) to identify systemic causes of human error during the RCA.
- Develop innovative corrective actions to enhance safety using the Human Factors Intervention Matrix (HFIX) and FACES tool.
You will receive:
- HFACS-RCA Handbook
- HFACS Interviewing Guide
- HFACS/HFIX/FACES Tools and Checklist
- ERAU Certificate of Completion
Continuing Education Units (CEUs)
Embry-Riddle will award 1.6 CEUs to each attendee who successfully completes the course.

Attendees Said...
"Its Amazing how the mindset can change and be positively redirected when structured theory is provided. The instructors were amazing as well as the material and coordination from ERAU. The leap of faith on virtuality was completely worth it."
"Very well done. ...the entire program was extremely well planned, well presented and highly informative."
"This was an excellent workshop. While I was aware of HFACS before the workshop I didn't have a strong enough understanding to apply it with confidence. I now feel the knowledge I have gained over the last two days has filled that gap."
"I thoroughly enjoyed the HFACS class and will be applying what I've learned immediately. I'm grateful for all the help and so appreciated Dr. Shappell and Dr. Wiegmann's instruction and experience. Well worth the time and money." U.S. Forest Service, Aviation Safety Manager
