Course Detail: OHFA - HFACS - Human Factors Analysis and Classification System (Virtual)

engine, man welding, doctor and patient, train on tracks

Overview

The Human Factors Analysis and Classification System (HFACS) is recognized as the global standard for conducting a thorough and credible human factors analysis of any adverse event. The HFACS system has direct application in accident investigation and analysis, risk reduction safety, quality and productivity improvement. Embry-Riddle Aeronautical University has teamed with HFACS, Inc. to provide online specialized human factors training to promote improved human performance and safety.

Human error is associated with 60 to 80% of all accidents, injuries, and quality defects across a variety of industries including aviation, healthcare, mining and manufacturing. Managing human error is therefore fundamental to maintaining the viability and profitability of any organization.

This intensive 2-day online virtual course provides training in the application of innovative methods for managing human error that are scientifically derived, empirically tested, and proven in the field. Participants will learn how to turn errors into information, information into knowledge, and knowledge into effective error management solutions.

The HFACS Workshop is also available in-person and held on the Daytona Beach Campus, Florida and in Las Vegas, Nevada.

Course curriculum and elements of this course may be applicable to meet some of the training requirements for eligible candidates to apply for the ICAP Federal Aviation Safety Officer Certificate (ASO). The ASO program is sponsored by the Safety Standards and Training Subcommittee of the Interagency Committee for Aviation Policy (ICAP).

The HFACS Course can be completed independently, or toward the Certificate of Management in Aircraft Accident Investigation

The launch of an HFACS-Advanced Course adds an exciting element to Embry-Riddle's training capabilities. Developed to meet the demands of evolving industry needs, HFACS-Advanced will provide training and implementation insight on enhanced proactive safety tools and data analytics and is recommended as a follow-on option to the HFACS Course.

Attendees will learn how to:

  • Integrate human factors and system safety concepts into the root cause analysis (RCA) or accident investigation process.
  • Utilize the Human Factors Analysis & Classification System (HFACS®) to identify systemic causes of human error during the RCA.
  • Develop novel corrective actions for improving safety using the Human Factors Intervention Matrix (HFIX) and FACES tool.

Attendees will receive:

  • HFACS-RCA Handbook
  • HFACS Interviewing Guide
  • HFACS/HFIX/FACES Tools and Checklist
  • ERAU Certificate of Completion

Continuing Education Units (CEUs)

Embry-Riddle will award 1.6 CEUs to each attendee who successfully completes the course.

Attendees Said...

"Its Amazing how the mindset can change and be positively redirected when structured theory is provided. The instructors were amazing as well as the material and coordination from ERAU. The leap of faith on virtuality was completely worth it."

"Very well done. ...the entire program was extremely well planned, well presented and highly informative."

"This was an excellent workshop. While I was aware of HFACS before the workshop I didn't have a strong enough understanding to apply it with confidence. I now feel the knowledge I have gained over the last two days has filled that gap."

"I thoroughly enjoyed the HFACS class and will be applying what I've learned immediately. I'm grateful for all the help and so appreciated Dr. Shappell and Dr. Wiegmann's instruction and experience. Well worth the time and money." U.S. Forest Service, Aviation Safety Manager
