Course Detail: LLCDSM - Evidence-Based Chronic Disease Self-Management Program
This FREE, evidence-based 6-session workshop is open to all community members to include individuals with diabetes, arthritis, fibromyalgia, asthma, COPD, heart disease, stroke, hypertension, kidney disease, cancer, osteoporosis, depression, and other such chronic health conditions. Each workshop session meets for 2 ½ hours once per week, and includes a 20-minute break. Participants learn how to apply self-management skills to reduce their chronic condition symptoms and associated problems such as tiredness, pain, feeling sad or anxious, and disruptions in daily routines, time with family and friends, and community engagements (church, volunteering, work, and/or educational activities). This workshop was developed and tested by Stanford University and studies show that participants have increased well-being, health-related quality of life, level of functioning, engagement in health-promoting behaviors, and confidence in their abilities to manage their chronic conditions. Additionally, studies have shown that workshop participants have decreased Emergency Department visits and hospital stays. Along with learning new self-management skills, participants have many opportunities to practice using their new skills as they give and receive supportive feedback from other participants. Some examples of workshop activities include the following: (1) learning how to use mind-based distraction techniques, positive thinking, relaxation exercises, better breathing, and improved sleep to decrease pain, tiredness, and difficult emotions that disrupt daily routines; (2) learning how to use healthy eating guidelines, exercise, and weight management to improve health outcomes, mood, and energy levels; (3) learning how to use communication skills, medication management techniques, and decision-making to more effectively partner with your healthcare providers; and (4) learning how to use action planning to set and meet your personalized health-related goals. This class is designed for individuals with ongoing health conditions and/or those who care for individuals with chronic health problems.
