DESCRIPTION:Event Description:\n\nServing Those Who Served: End-of-Life Care for Veterans \n\nThis presentation equips hospice professionals with the knowledge and practical tools needed to provide end of life care that meets the unique needs of veterans. Participants will gain a deeper understanding of military culture through the "15 Things Veterans Want You to Know" and explore the experiences of veterans who served in American combat eras over the past 85 years. The session will introduce evidence-based screening tools to identify veterans' physical, psychosocial, and spiritual needs and demonstrate how to integrate these assessments into the hospice admission process and individualized plan of care. Attendees will also learn best practices for addressing challenges such as PTSD, moral injury, and meaningful acknowledgment of military service, while exploring programs to honor veterans at end of life.
